2. SMS/Text Message Communications

By providing your mobile phone number and checking the SMS consent box on our patient intake form, you expressly consent to receive automated text messages from Concierge Physical Therapy at the number provided. These messages may include:

•       Appointment reminders and confirmations

•       Follow-up care check-ins (typically 3 weeks after your last visit)

•       Secure patient portal access links

•       General care-related communications from your physical therapist

 Message Frequency: Message frequency varies. You may receive up to 4 messages per month.

 Standard Rates: Message and data rates may apply depending on your mobile phone service plan.

 Opt-Out: You may opt out of SMS communications at any time by replying STOP to any message. You will receive a confirmation: "You have been unsubscribed from CPT reminders. Reply START to resubscribe." For help, reply HELP to any message.

 Consent is Not Required: Consent to receive text messages is not required as a condition of receiving physical therapy services.

 We do not sell, share, or disclose your mobile phone number or SMS consent information to third parties or affiliates for marketing purposes. Your phone number is used solely to deliver the communications described above.

3. Use of Your Information

We use your information to:

•       Schedule and manage your physical therapy appointments

•       Provide and coordinate your physical therapy care

•       Send appointment reminders and follow-up communications

•       Comply with HIPAA and applicable healthcare regulations

•       Improve our services and patient experience
