Do I Need a Referral for Physical Therapy in Florida?

Short answer: No. In Florida, you can see a licensed physical therapist directly no doctor's referral required. If your shoulder is aching, your back went out, or you've been dealing with nagging knee pain, you don't need to make a separate appointment with your primary care physician before getting treatment.

What Is Direct Access to Physical Therapy?

Direct access is the legal right for patients to seek evaluation and treatment from a physical therapist without first obtaining a prescription or referral from a physician or other healthcare provider.

Florida adopted direct access in 2000, and the law has been in place for over two decades. As a patient in Pinellas County, Clearwater, Largo, or anywhere in the state, you can call a PT clinic today and book an appointment without visiting your doctor first.

Who Can Use Direct Access in Florida?

Most adult patients can. Florida's direct access law allows licensed physical therapists to evaluate and treat patients without a referral for an initial period of 30 days.

After 30 days of treatment, if you still need ongoing care, Florida law requires a referral from a licensed healthcare provider (physician, nurse practitioner, physician assistant, etc.). In practice, for the vast majority of musculoskeletal conditions shoulder pain, back pain, neck pain, joint stiffness, sports injuries 30 days is more than enough time to achieve meaningful results.

At Concierge Physical Therapy, most patients complete their plan of care in 3 visits or fewer. The 30-day window almost never becomes a factor.

Why Do So Many People Think They Need a Referral?

There are two main reasons:

1. Insurance requirements are different from the law.
If you're using health insurance to pay for physical therapy, your insurance plan may require a referral before it will cover visits even though Florida law doesn't. This is a payer policy, not a legal requirement.

At Concierge Physical Therapy, we're a cash-pay practice. We don't bill insurance, so there are no prior authorization delays, no referral requirements from your insurer, and no waiting weeks to find out if your plan will cover care. You schedule, you come in, you get treated.

2. Old habits and assumptions.
Many patients have always gone to their doctor first out of habit, or because that's what they were told years ago. The direct access law changed this but the assumption stuck.

What Conditions Can a PT Treat Without a Referral?

Physical therapists are trained to evaluate and treat a wide range of musculoskeletal, neuromuscular, and movement-related conditions. Common reasons patients come directly to us include:

If you're unsure whether physical therapy is appropriate for what you're dealing with, we'll tell you honestly during your evaluation. If something requires imaging or a physician's input first, we'll say so.

When Should You See a Doctor First?

Direct access is not a substitute for medical evaluation when something more serious may be happening. See a physician before coming to PT or alongside PT if you're experiencing:

  • Unexplained weight loss combined with back pain

  • Pain that came on with no injury and has been worsening for weeks

  • Numbness, tingling, or weakness that is severe or rapidly progressing

  • Signs of infection (fever, redness, heat around a joint)

  • A recent fall or trauma where fracture is possible

  • Bowel or bladder changes associated with back pain (seek emergency care immediately)

These are red flags that warrant medical workup. For everything else, the typical aches, strains, stiffness, and injuries that make up the vast majority of PT referrals you're safe to come directly to us.

  3. Leave with a plan — most patients know what's wrong and what to do about it after the first visit

  4. Get results — our average patient completes care in 3 visits or less

We see patients at our primary clinic in Largo, FL (Mon–Fri 9am–6pm, Sat 9am–1pm) and at our St. Petersburg location (Wednesdays 10am–1pm, 420 94th Ave N).

Can I Still Use My Insurance Referral If I Have One?

Yes, but it won't change anything about your care at our clinic. We're a cash-pay, out-of-network practice, which means we don't accept insurance directly. If your plan has out-of-network benefits, you may be able to submit a superbill for partial reimbursement, we can provide that documentation.

Many patients find that the cost of cash-pay PT, with faster results and no wasted visits, is comparable to or less than the total out-of-pocket cost of going through insurance with copays, deductibles, and the time spent on prior auth delays.

Using an HSA or FSA? Physical therapy is an eligible expense. You can use those funds directly with us.
